  1. How long does it take to be credentialed?
    Most credentialing can be completed in three to six months
  2. How do I apply for a status change?
    Status changes must be submitted in writing to Medical Affairs. Link to Status Change Form (pdf)
  3. How do I apply for privilege modification?
    A written request for modification to privileges and documentation of training is required. This is reviewed by the Chief of the Department and recommended to the Credentials Committee. Link to Modification of Privileges Form (pdf)
  4. How do I obtain parking?
    Medical Staff with “Active” status, who identify Crouse as their primary hospital are eligible for parking. Contact Medical Affairs for a Parking Permit. Link to instructions.
  5. How do I access dictation and the SMS system?
    Contact Medical Affairs Department for and ID # and password for Transcription and SMS system. Link to SMS Instructions (pdf).
  6. How often is reappointment and what are my responsibilities?
    Reappointment is every two years. You are responsible for completing your reappointment application, a delineation of privilege form, and copies of any licensures, certifications, or malpractice insurance that may have expired.
  7. How do I apply for a leave of absence?
    Voluntary leave of absence by a Medical Staff Member may be granted for good and sufficient reason by the Board of Directors on recommendation of the Department Chief. The Medical Staff Member’s written request shall include the reason for the leave and any material necessary to properly evaluate the request. Link to Leave of Absence Form (pdf).
  8. What do I need to do if I relocate or move?
    A resignation letter should be sent to Medical Affairs to notify us of the date you will be ending your affiliation with Crouse Hospital. A forwarding address should be included.
  9. Can I review my quality file?
    Yes, you can review your quality file. It must be reviewed in the presence of a Medical Affairs staff member. No copies can be made, but you can take notes.
