Crouse Hospital needs initial paperwork, including information required by state and federal law, to be completed before you begin working. To access and complete these forms, enter the username and password that was created when you submitted an application. Please complete these forms before coming to your New Hire Paperwork Appointment with your recruiter in Human Resources.
To enroll in the Crouse benefit plans, you will need to supply us with
- Social Security numbers and birth dates of dependents who will be enrolled in benefit plans;
- A copy of your Marriage Certificate if you are enrolling your spouse in any benefits plans
- A copy of Birth Certificates for your dependent children.
Please bring these documents with you to your New Hire Paperwork Appointment.
