Our Team

Judith Hamlin Judy Hamlin, MS, FNP, RN, CCM - Clinical Nurse Manager/Interim Director
A master’s prepared family nurse practitioner focused on geriatrics, Judy Hamlin was formerly with Loretto’s PACE program and the Syracuse Veterans Administration. She has experience in complex case management; disease management; inpatient psychiatric care; and respiratory care. A member of the Omicron chapter of Sigma Theta Tau, Hamlin received geriatric scholarships in 2010 and 2011 from the Community Health Foundation of Western and Central NY. She is facilitator of the Crouse ACE (Acute Care for the Elderly) team and leads the hospital's Geriatric Resource Team.

Dube

David Dube, MD - Physician Advisor
David Dube, MD, is the physician advisor to the Aging and Complex Care Service at Crouse Hospital. A board-certified internist with a two-year fellowship in geriatrics, Dr. Dube was one of the first 60 certified medical directors in the U.S. and one of the first with a geriatric certification. Dr. Dube also serves as medical director of St. Camillus Health and Rehabilitation Center in Syracuse. In addition, he is on the faculty of SUNY Upstate Medical University and a staff physician at Hutchings Psychiatric Center.


Sharon-Martin

Sharon Martin, BSN - Clinical Program Coordinator, Transition Coach
With extensive experience in electronic medical records and program coordination, Sharon Martin is a bachelor's prepared nurse with experience at the bedside in the general medical-surgical unit. Martin is an active participant in the national HELP (Hospital Elder Life Program) initiative, sharing the Crouse senior volunteer program experience with colleagues around the country. She also serves as a Transition Coach for Crouse Hospital's Care Transitions Program, which was developed by Eric Coleman, MD, MPH, of the University of Colorado.


Diane-Nanno

Diane Nanno, BSN - Transition Coach
Diane Nanno is a bachelor's prepared nurse with 25 years of experience in home healthcare. Before joining Crouse, she led the development of the Heart Smart Program at the Visiting Nurses Association of Central New York. Nanno also serves as the Northeast Regional Coordinator for the Care Transitions Program, directed by Eric Coleman, MD, MPH, and is a consultant for Care Transitions training for the Community Health Foundation of Western and Central New York. She's a member of the American Association of Heart Failure Nurses and the Omicron chapter of Sigma Theta Tau.
