Free Weight Loss Surgery Information Seminars

To attend one of our surgical weight loss information seminars, simply

- Register for that seminar by completing the form below or by calling 315/472-2464.

Free parking is available in the Marley Education Center garage (beneath the building, with entrance located at the corner of Irving Ave. and University Pl.)

 


 

At the seminar, you'll be asked to complete a simple, one-page evaluation form. Our team will review the information and will call you within five business days to schedule an appointment with Dr. DeSimone or Dr. Cooper.

* Does your health insurance cover weight loss surgery?

It is important that you contact your health insurance carrier BEFORE registering for the seminar to see if they have excluded weight loss (bariatric) surgery from your coverage. Crouse is currently not listed as a COE for Medicare and Medicaid. Therefore, we are not able to accept these insurances at this time. Please check back at a later date for updates.
