How to Apply
Application Process
Students who want to apply to Crouse Hospital School of Nursing must:
1. Mail the completed application form and application fee to:
Admissions Office, Crouse Hospital School of Nursing, 736 Irving Ave., Syracuse, NY 13210.
2. Arrange for the following required documentation to be sent to the admissions office:
- Official copy of high school transcript or
GED record (required even if the applicant has attended college);
- Official copies of all college/postsecondary
transcripts;
- Official copies of standardized test scores
(SAT, ACT, TOEFL) as indicated;
- Two completed reference forms. References should be professional or
acedemic in nature. Letters of reference for international applicants
must be from individuals residing in the United States. References
from friends or family members are not acceptable.
Application materials become the property of Crouse Hospital School of Nursing, and the school cannot return them to applicants.
It is each applicant’s responsibility to ensure that the application folder is complete. Students may call the admissions office at (315) 470-7481 to check its status.
All application requirements must be completed & received (NOT postmarked) PRIOR to the deadline dates.
Application Deadlines:
Fall Semester, day session - February 1
Fall Semester, evening session - June 1
Spring Semester - September 1
