Admissions
A centralized intake team, staffed by credentialed chemical dependency counselors, works with individuals and referral sources to carefully evaluate the level of care required and to arrange for admission to the program most appropriate for the patient’s needs.
Patients requesting outpatient care are scheduled for an intake appointment with a counselor at one of our outpatient locations, where an in-depth assessment interview is conducted. After a decision has been made as to the level of care required, all referral sources are notified via letter about the determination for the patient. The letter describes:
- Recommended program
- Start date
- Program hours of operation
- Other pertinent information
If a patient’s insurance requires pre-certification, our team will complete this step prior to treatment.

Opioid Treatment Program (Methadone Maintenance)
Crouse Hospital is the only Central New York provider to offer a methadone maintenance treatment program. Methadone maintenance is the treatment of choice for patients who require medication to remain abstinent from illicit opioids.
Crouse’s Opioid Treatment Program is designed for patients ages 18 and older who are addicted to opiates such as heroin, codeine, hydrocodone, oxycontin and others. The treatment plan addresses all addictions, medical issues, psychiatric issues and psychosocial problems.
Counseling and medication are provided according to state and federal regulations, as well as the individual needs of the patient. Upon discharge, aftercare is established by referral.
To qualify for the program, patients must meet state and federal criteria for type and duration of addiction. Extensive assessment is done with back-up documentation required to validate a history of addiction. A physical examination also is performed to establish current addiction.
Cost/Coverage
Most private insurance, Medicare and Medicaid are accepted. A sliding fee scale is available for those without insurance.

Recovery Challenge Intensive (RCI) Program
The Recovery Challenge Intensive (RCI) Program meets the regulatory guidelines for Day Treatment Rehabilitation. The program provides treatment services for adult male and female patients who meet criteria for outpatient rehabilitation substance abuse treatment and present with one or more of the following conditions:
- Traumatic brain injury
- Learning disability
- Developmental disability
- A history of special education
The program makes many adjustments to assist individuals in gaining independence and maintaining abstinence, such as:
- Frequent phone contact
- Case management
- Ongoing and open communication with other service provider
- Advocating
- Coaching
Treatment components are facilitated in a very concrete, easy-to-understand manner. Counselors use approaches appropriate to the group’s level of functioning.
RCI patients participate in:
- Group therapy
- Chemical dependency education groups
- Vocational and leisure planning
- Individual meetings with primary counselors, as prescribed by the treatment team.
Patients meet Monday through Thursday for four hours/day. Daily lunch is provided.
Unlike any other treatment program, RCI offers a transition period into the Recovery Challenge Continuing Care (RCCC) program, which supports patients’ adjustment to change and provides stability. As part of the stepdown process, patients attend two weekly groups in both RCI and RCCC.

Inpatient (Hospital-Based) Programs
Healthcare Intervention Services (HIS)
Crouse Hospital's Healthcare Intervention Service (HIS) identifies substance abuse problems in patients admitted to the hospital for other medical reasons.
Often the comprehensive care of a hospital stay can be a valuable opportunity to identify and address chemical addiction. HIS staff members use a special questionnaire to assess a patient’s potential for problems with drugs and/or alcohol. Individual assessment, intervention and follow-up services are provided.
Physicians and nursing staff refer patients to the program. HIS works with patients and health care providers to make the most appropriate referral for treatment upon hospital discharge.

Detoxification Program
The Detoxification Program at Crouse Hospital is designed for those who require medical services for withdrawal from alcohol or other substances. An interdisciplinary team of healthcare professionals work with each patient to provide individual and group therapy as an introduction to treatment. Working closely with a counselor, patients receive referral for further treatment upon discharge from the hospital.

How to Access the Program
Patients who may possibly require detoxification are evaluated at 410 S. Crouse Ave., just down the hill from the hospital, during normal business hours. After hours and on the weekends, patients are evaluated and referred to the program by Emergency Department personnel.

Commonwealth Place Residential Program
Commonwealth Place is a 40-bed residential center dedicated to the treatment of adults age 18 and older with chemical dependency who will benefit from 24-hour per day treatment in a supervised setting.
Treatment is based on the individual needs of each patient and includes:
Family involvement also is emphasized.
Because discharge to a less restrictive environment is the goal, patients are evaluated daily to determine when they are ready to continue treatment through one of Crouse’s many outpatient programs. Before discharge, however, patients participate in comprehensive aftercare planning that includes:
Choosing an appropriate place for continuing treatment
Making a plan to go back to work
Planning to attend Alcoholics Anonymous, Cocaine Anonymous, and/or Narcotics Anonymous meetings

Outpatient Programs
Primary Addiction Treatment
Crouse Hospital’s Primary Addiction Treatment Groups are designed for adults experiencing chemical dependency and its consequences (legal or otherwise) as a result of addiction.
The group meets weekly and focuses on the disease concept of addiction, including:
- How addiction affects the family
- Self-esteem
- Stress management/coping skills
- Introduction to self-help
- Spirituality
- HIV/AIDS education
- Sober social network and leisure activities
Relapse Prevention Treatment
Relapse, slips and flare-ups are a natural part of the recovery process. Adults in recovery can benefit from additional support at key times during the process. Prior treatment is a pre-requisite.
Crouse Hospital’s Relapse Prevention Program meets the needs of recovering adults who:
- Recognize signs and symptoms of imminent return to use
- Are stable in recovery but seek to strengthen relapse prevention skills
- Have recently relapsed and seek a return to a recovery lifestyle
Staffed by a specialist in relapse prevention treatment, a conveniently scheduled group format is used to accommodate those who work or provide primary dependent care.
Working toward the goal of sustained recovery, counselors utilize:
- Structured, skill-building education
- Role playing
- Trigger identification
- Other cognitive behavioral approaches

Outpatient Rehabilitation Services
Day Treatment
Crouse’s Day Treatment Rehabilitation Program for adults age 18 and older targets those struggling to abstain from chemical use. Treatment includes a daily therapy group session and individual counseling sessions as prescribed by the treatment team, along with:
- Activity therapy
- Vocational exploration
- Leisure skills education
- Chemical dependency education
The goal of Crouse’s Day Treatment programs is to assist patients in returning to a sober, productive lifestyle. Patients meet for five hours, Monday through Friday. Lunch is provided daily.
Upon completion of the Day Treatment Rehabilitation level of care, patients will go on with treatment in a continuing care group, three times per week for 2 ½ hour sessions. This will allow patients to pursue vocational and educational plans.
Self-help group participation in Alcoholics Anonymous, Cocaine Anonymous and/or Narcotics Anonymous is encouraged.
Upon completion of the program, patients often are referred to an aftercare group and are strongly encourage to maintain self-help group involvement for continued success.

Outpatient Adolescent Services
Crouse’s Outpatient Adolescent Program serves young people between the ages of 13 and 18, who are chemically dependent. It is staffed by specialists in chemical dependency and development.
Both group and individual therapy is provided. Participation in self-help groups is rencouraged.
Aftercare services reinforce the young person’s commitment to a chemical-free life.
The program includes:
- Group and individual counseling and support
- Expressive therapy
- Chemical dependency education
- Development of sober living skills

Evening Treatment Program
Crouse's Evening Treatment Program is an intensive outpatient experience designed for chemically dependent patients who:
- Work
- Attend school during the day
- Are primary caretakers of young children or elderly parents
The program meets three times each week for two-and-a-half hours.
Individual treatment, group therapy, education, and participation in self-help groups are required to help reinforce the treatment and rehabilitation process.
